Taxable Income Reporting

Departments must complete the Taxable Income Reporting Form for gifts, prizes, and awards of $100 or more (or any amount for non-resident aliens) in accordance with the Gifts, Prizes, and Awards Policy and the corresponding Gifts, Prizes, and Awards Funding Source Allowability Chart. This data will be used to sum the value of all gifts, prizes and awards to one individual.

Completing the Form

Complete all required fields and click Submit. 

  • A copy of the submission data will be emailed to the department contact's email address provided on the form. This email and the related tax information (W-9 or W-8) must be maintained in the department.
  • For reimbursement requested from UBF funds, a copy of the W-9 (or W-8 if the recipient is a non-resident alien) must be submitted with the reimbursement request.
  • The compliance office will contact the department at the end of the year to obtain information needed to complete required taxable income reporting forms to be sent to the Internal Revenue Service (IRS). 
Requesting Reimbursement?

A copy of the submission email must be submitted with all reimbursement requests.
