Relocation Services Request for Employees

Departments must notify Human Resources when they have made an offer of employment to a new employee that includes financial support for an eligible relocation.  

Do Not Use This Form If:
  • The relocation is for an international move 
  • The employee is not a U.S. citizen or permanent resident of the U.S.

Instructions

Only one departmental contact should complete this request. Multiple requests may delay an employee's relocation.
  1. Complete all required fields.
  2. Attach a signed copy of the employee's offer letter (the letter must include the relocation services clause).
  3. Click Submit Request.
