Raffle Proposal Request

A university department, entity or organization planning to conduct a raffle must complete this form and receive approval prior to the raffle being conducted. This form must be completed per the Raffle Policy For University Fundraising Efforts.


  1. Complete all required fields and click Submit Request.
  2. You will receive a confirmation email at the email address you provided on the form.
  3. Financial Management will upload your request to Adobe Sign to obtain all necessary signatures.
