COVID Religious Exemption Form

Students requesting a religious exemption are required to submit a completed and signed COVID-19 Vaccine Religious Exemption Request Form to UB Health Services. Inclusion of general philosophical, political, medical or moral objections to the COVID-19 vaccine will  result in a denial  of a religious exemption.

Students who receive an approved  religious exemption should be aware that some off-campus placements may have stricter vaccination requirements than the university and do not accept UB’s exemptions. In these instances, students who receive a UB-approved religious exemption may not be able to complete their degree.
