The University at Buffalo Information Technology (UBIT) operates as a covered entity as defined by the U.S. Department of Health and Human Services Office of Civil Rights. HIPAA Regulation Text 45 CFR Part 164.310(a) requires a covered entity to implement policies and procedures that limit physical access to its electronic information systems and the facility or facilities in which they are housed, while ensuring that properly authorized access is allowed.

Policy

UBIT implements procedures that permit the authorized access to its facilities by its workforce members in order to support restoration of lost data in the event of an emergency, as described in the Disaster Recovery Plan and Emergency Mode Operations Plan Policy.

Applicability

This policy applies to all UBIT workforce members.

Responsibility

Workforce members: Adhere to all policies and procedures as written.

HIPAA Security and Privacy Officer: Ensures authorized access to ePHI in the event of an emergency.

Compliance Officer: Participates in ensuring security of ePHI is effective and enforced, in conjunction with the HIPAA Security and Privacy Officer.
