Faculty or staff request pickup of hazardous materials waste using this form.

Instructions

When hazardous waste containers in laboratory satellite accumulation areas are no more than 90% full:

  1. Complete Date When Full on the container's hazardous waste label.
  2. Download and complete the "Hazardous Waste Disposal Request" using the instructions on page 2 of the form.
  3. Scan the form and email to hazwaste@buffalo.edu
