Agency Account Request

Agency accounts are used to receive, hold and disburse funds on behalf of students, faculty, staff members or appropriately recognized organizations. For additional information on Agency Account eligibility, refer to the SUNY Agency Account Guidelines.


  1. For each request complete all the fields for each record. Required fields are marked with a red asterisk.
  2. Submit the request.
  3. Check your email for the account request form. Step by step instructions will be included in the email.
