- VPAT – Voluntary Product Accessibility Template - A template containing information regarding how an Information and communications technology product or service conforms with Section 508 of the Rehabilitation Act of 1973
- HECVAT – Higher Education Community Vendor Assessment Toolkit – A questionnaire developed by EDUCAUSE specifically for higher education to measure vendor risk - (replaces UBIT Vendor Questionnaire). Note: UBIT Vendor Questionnaire (see below) may be used if vendor does not have HECVAT.
Submitting the form
Remedyforce ticket, form and additional documentation must be submitted by IT Staff only.

- Attach the completed Vendor Assessment Request (VAR) form
- Attach additional documentation
