Additional vendor documentation

  • VPAT – Voluntary Product Accessibility Template - A template containing information regarding how an Information and communications technology product or service conforms with Section 508 of the Rehabilitation Act of 1973
  • HECVAT – Higher Education Community Vendor Assessment Toolkit – A questionnaire developed by EDUCAUSE specifically for higher education to measure vendor risk - (replaces UBIT Vendor Questionnaire).  Note:  UBIT Vendor Questionnaire (see below) may be used if vendor does not have HECVAT.

Submitting the form

Remedyforce ticket, form and additional documentation must be submitted by IT Staff only.

  1. Open the Remedyforce link

  2. Select Request a Service
  3. Select Applications and Software > Software and Web-based Services Vendor Assessment review request
  4. Attach the completed Vendor Assessment Request (VAR) form
  5. Attach additional documentation
