New York State law requires the consent of a parent/guardian for medical care (reproductive and sexual health care excluded) of persons under 18 years of age.

Policy Statement

If a student is younger than 18 years of age during their time as a UB student, a parent or guardian must provide written consent for treatment. Students under 18 cannot access health care from Student Health Services without a completed Parental Consent Form on file. This form must be completed if the student is under 18 years of age during their time as a UB student, even if the student will turn 18 during the academic year.

Procedure

Prior to making an appointment with Student Health Services, or receiving any type of care, a parent or guardian of a student under the age of 18 years old must fill out the Written Consent Form and submit it via mail or fax.
