Social Security Number (SSN) Access Request Procedure

  • Before completing the form, you must complete the Handling Data Safely Training on UBlearns.
  • It is advised that you fill out this form with your supervisor, as you will need their approval/signature.
  • Your SSN access is revoked when you change roles on campus. Departments are responsible for informing us when employees change roles or leave the university. You must re-submit the online form for SSN access in your new role.
  • SSNs are categorized as Category-1 Restricted data. You are expected to be in compliance with the security standard in accordance with the Data Risk Classification Policy. Compliance will be checked annually in coordination with your IT support.

Form review by the Social Secuirty Number oversight committee occurs within three business days. If the committee has questions, they will contact you directly.
