Sending Your Packet

Prehealth Advising will upload your packet to the central application services after all letters are received, you’ve finalized your letter list, and you’ve submitted the release form at the bottom of this page. We cannot send an applicant’s letter packet until we receive a release form. You cannot submit the release form until you have opened your central application, as you will need certain applicant ID numbers. 

If you previously had an evaluation letter from the Prehealth Committee and are now interested in having your packet sent to an application, please visit our Prehealth Committee Reapplicants page for information on how to request an addendum to your Committee packet. 

If you would like your packet sent to a military recruiter for consideration for the Health Professions Scholarship program, or to a post-bac program, you must also submit a release form.

This policy applies to all applicants within any cycle.

As a reminder, you must provide a separate form for each application you want us to send your packet to.

We will need additional information based on what you are applying to:

  • AMCAS - List BOTH your AAMC ID and the AMCAS Letter ID.
  • AACOMAS, AADSAS, OptomCAS, AACPMAS, or TMDSAS - List your application ID.
  • International School - List the school name, contact person (if you know it), and email address for submission.
  • Post-bac/Graduate Program - List the school name, program name, contact person (if you know it), and either submission method (ex. PostbacCAS) or email address for submission. 
  • Military program/Scholarship - List the name of the recruiter and email address for submission.
  • EOPIM - No additional information is required so please just enter "N/A".
