Summer Food Service Program - Income Eligibility Form

Carefully follow the instructions below to complete income eligibility form in order to qualify for the summer food service program.

Income Eligibility Form Instructions

  1. Open the Income Eligibility Form (PDF) in a new browser tab.
  2. Read the entire set of instructions on page 1 of the form.
  3. Complete parts 1-5 according to the instructions on page 1.
  4. Once you complete the PDF form, download and save a copy of it on your local machine.
  5. Then come back to this page and complete the web form below.

The estimated time to complete this form is between 5 and 15 minutes based on having all of the information required in the form. The time estimate does not include preparation and collection of the data, information and documents needed for form completion.
