Teaching technology vendors should use this form to register with UB. A vendor registration will provide administrative, department and program operations a single contact point and source of information about technologies: whether they are implemented at UB, if there are any recommendations for use of the technology and any additional vendor information required for procurement and implementation of a technology at UB.
In the second sedtion of the form you will be asked to identify the features, pedagogical themes, and methodologies of your teaching technology. In the last section of the form you will be required to agree to, provide details on, or upload a number of documents. Please read through and review the following lists before completing the vendor registration form
You will be asked to selet the kinds of student information tor course data the application needs to access or exchange.
You will be asked to select the types of faculty or student costs or purchases required to access/use your application and then provide information about those costs or requirements.
You will be asked if there are any signatures, click-through terms or hyperlinks required for a quote, order forms, or SOW, and if so to provide the terms that need to be accepted (MSA, general terms, privacy policy, etc) even for a no-cost or free-to-access service.
You will be asked to accept the NYS Exhibit A without edits or redlines.

You will be asked if you require faculty or students to accept a hyperlink or click-through term to access or use your application, and if so prompted to upload a document with hyperlinks and Click-through terms presented on any quotes, order forms, or SOW.
The LTI Requirements document is required for our university review process. If you are not able to upload this required document you will be asked provide an explanation of how your technology is meeting the requirements and provide links to download the supporting materials.
The VPAT/Accessibility Conformance Report is required for our university review process. If you are not able to upload this required document you will be asked to provide an explanation of how your technology is meeting the requirements and provide links to supporting materials.
You will be asked to provide pricing models and associated user fees for faculty and students, including any registration or paywall requirements. The Pricing models and associated user fees document is required for our university review process. If you are not able to upload this required document yo will be asked to provide an explanation of how your technology is meeting the requirements and provide links to download the supporting materials.
The HECVAT Full Report or UB Vendor Questionnaire for IT Purchases is required for our university review process. If you are not able to upload one of these required documents you will asked to provide an explanation of how your technology is meeting the requirements and provide links to download the supporting materials.
