Mobile Communication Device Agreement

An employee's supervisor initiates a document describing the business case for an employee's UB-supported mobile communication device. The employee agrees to the terms and signs the agreement.

The Mobile Communication Device Agreement documents the supervisor's approval and employee's agreement to the terms of receiving a department-owned mobile communication device or an allowance for a personally-owned device. Business need must be reviewed and approved annually by the employing department.
