Independent Contractor Agreement

The Independent Contractor Agreement is required when an independent contractor is providing goods or services valued at more than $50,000. Purchasing and Contract Services will sign the agreement on behalf of the Research Foundation.

Completing the Microsoft Word Form

  1. Fill in the required information in the appropriate fields
  2. Obtain the independent contractor's signature
  3. Do not sign the agreement; Purchasing and Contract Services will sign on behalf of the Research Foundation
  4. Attach the completed agreement with independent contractor's signature to your purchase requisition
