Consent is required from students 18 years old and over, before Student Health Services can release any medical information to another person.
If a patient is 18 years of age or older we cannot release any medical information to a parent, spouse, instructor, employer or anyone else other than the patient themself. Patients who would like Student Health Services to verify an appointment or discuss diagnosis and treatment with anyone other than themselves must sign an authorization consent form during their visit. This consent pertains to that day’s appointment only; a new release form must be signed for each visit authorized for release.
Notify Student Health Services who they are permitted to release your medical information to.
During your visit, sign an authorization consent form if you wish to permit Student Health Services to share protected health information with another individual. This form must be completed for each visit authorized for release.
To maintain confidentiality, contact your health insurance company and ask that any ‘Explanation of Benefits’ involving you be sent directly to you and no one else.
