Allied Health students who come to the building for classes or activities are strongly encouraged to get COVID-19 boosters when eligible, in accordance with SUNY guidelines. Students enrolled in any program or course, or who are participating in a clinical or internship at an off-campus location, must comply with all health and safety guidelines in place at the location, including any vaccination or testing protocols.
New and continuing students must submit documentation of their COVID-19 primary vaccination(s) before they are allowed to register for courses. If you have not already done so, submit proof below.
The Excelsior Pass is not sufficient proof of vaccination.
The SUNY requirement recognizes vaccines that have been authorized/approved by the U.S. Food and Drug Administration (FDA) or authorized by the World Health Organization (WHO).
Students who hold genuine and sincere religious beliefs which are contrary to COVID-19 vaccination may be exempted from this requirement. General philosophical or moral objections to the COVID-19 vaccine will not result in approval of a religious exemption.
Students requesting a medical or religious exemption must download and submit a completed COVID-19 Vaccine Medical or Religious Exemption Request Form.
