Request a waiver of the measles, mumps and rubella immunization requirement if you are registering for online courses only, and you will not physically attend classes on any of UB’s campuses.
If you did not yet read the informational immunization and meningitis requirements, please do that before completing and submitting this form.
Students who are completely remote can submit this waiver form each semester that they are taking classes exclusively online. The waiver enables us to remove your IMM hold through the last day of drop/add for that semester. You will need to complete a new waiver for each semester that you are completely online.
