Request / Order a Transcript

Students may request to have official transcripts of their UB academic work sent to themselves or designated third parties. Unofficial transcripts are available through HUB for students who attended UB since spring 2011.

Official Transcripts

Transcripts Are Sent “As Is”

Students are responsible for ensuring that their transcript is ready to send at the time of making their request. Students can verify grades, final GPA, and/or degree conferral by viewing a copy of their unofficial transcript first. Term and Cumulative Statistics will not be included on the official transcript until after the grading deadline for the term. Students may also select the “Hold for Grades” or “Hold for Degree” option when placing an order in Parchment.

Transcripts will be released only upon requests signed by the student on pen and paper or electronically using mouse, stylus or finger signature technology. Additionally, transcripts may be released without signature when identifying information is provided and confirmed during the electronic ordering process.

It is against university policy to email or fax student records, since these are not secure forms of communication. Students who wish to send their transcripts electronically should utilize Parchment.

Holds May Prevent the Release of Official Transcripts

Current or former students who have holds that prevent the release of their transcript will not be able to receive a copy of their official transcript. Financial holds do not prevent the release of official transcripts. Requests will be reviewed by the Office of the Registrar and processed accordingly.

Students Who Attended UB 2003 - Present

Students can order an electronic or paper transcript online through Parchment.

  • Orders include expedited processing, order tracking and secure delivery. 
  • Students can include attachments with their transcript requests as needed (e.g., AMCAS, PharmCAS, Medical or Law School application forms, etc.).

Fees

An electronic transcript ordering fee of $7.50 will be assessed. The fee is inclusive of a $2.60 processing fee charged directly by the vendor and a $4.90 campus surcharge for administrative support of transcript production including print and postage for hard copy transcript orders. Alternative shipping options are available at cost to the individual.

Complete the transcript request form to receive a free paper copy of an official transcript. Forms must be sent by fax or mail, and will not be accepted by email.

Requests will be processed in 7-10 business days, and transcripts will be sent via USPS with no tracking information. The Office of the Registrar is not able to provide paper transcripts via email, fax or an expedited shipping service such as UPS or FedEx.

Students Who Attended UB Before 2003

Students whose entire enrollment at UB was prior to 2003 have the following options.

Records Prior to 1993: Contact the Office of the Registrar for Availability

Records prior to 1993 may not be readily available for in-person pickup. If the student’s attendance was prior to 1993 and they intend to pick up their transcript at 1Capen, please contact the Office of the Registrar with the student’s name at the time of enrollment and date of birth to verify pickup availability.

Dental, Law and Medical Students

Records for the School of Dental Medicine, School of Law and Jacobs School of Medicine & Biomedical Sciences are maintained within those schools. Current or former dental, law or medical students should contact their respective schools to obtain academic transcripts.

Unofficial Transcripts

Unofficial copies of transcripts are available in HUB for students who attended UB since spring 2011. If students have a preferred/chosen name designated in HUB, it will print on their unofficial transcript.

Apostille: Authenticate a Transcript or Diploma for Use in a Foreign Country

An apostille refers to the legalization of a document for international use. The apostille certifies the signature and the position of the official who has issued or certified a copy of a document.

Students may need an apostille to authenticate their transcript and diploma for use in a foreign country. Apostilles are only valid in countries that joined the 1961 Hague Convention.

The Office of the Registrar assists students with this process by notarizing the document(s). Students then follow the steps below with the Erie County Clerk and New York State Secretary of State offices to complete the apostille.

Note that an apostille diploma can only be provided if the diploma is printed with the student’s legal full name.

Step 1: Request documents from the UB Office of the Registrar

1a. Request document(s) that need an apostille

Diplomas

Photocopies of diplomas are preferred so that the original diploma does not get lost or damaged. If students are not in possession of their original diploma or do not have a copy of their original diploma, they may purchase a replacement diploma. Please note there is an additional fee for each replacement diploma, and orders will take an additional week to arrive in our office. Diploma apostille can only be provided if the diploma is printed with the student’s legal full name.

Transcripts

1b. Prepare a written request for an apostille

The written request must include the following information

  • Name
  • Date of birth
  • Phone number
  • Email
  • Country where the apostille is to be used
  • How the document(s) should be returned (pick-up on campus, or mailed to a specific address)
  • If notarized documents are to be released to a third party, include their full name. The third party must present photo ID when picking up documents.

Office of the Registrar’s Responsibilities

  • Provide the original signature of the Registrar administration on documents.
  • The notary public attends to the authenticity of the signature appearing on the transcript(s) and/or original or duplicate diploma(s).
  • The transcript and/or diploma will be embossed with the University at Buffalo’s raised seal.
  • Documents will be returned to the requestor or authorized third party.

The Office of the Registrar does not issue the apostille.

Step 2: Send documents to the Erie County Clerk's Office

After documents have been notarized, the notary public signatures must be authenticated by the Erie County Clerk’s Office.

There is a $3 fee per authentication. For example, if a student requests a transcript and a diploma, they will pay a $6 fee to the Erie County Clerk’s Office.

A money order or certified check made out to the Erie County Clerk must be provided to process the request. There is a $3 fee per document. Only money orders or certified checks are acceptable by the Erie County Clerk and all fees must be paid in U.S. dollars.

If the documents are mailed to the Clerk’s Office, the student can include a postage-paid envelope addressed to the Department of State, the $10/document fee, the Request Form to the Department of State and a return envelope to the student’s address. The Clerk’s Office will then mail the certified documents to Department of State on the student’s behalf.

Step 3, Option 1: Bring documents to New York Department of State

Step 3, Option 2: Mail Documents to New York Department of State

If the student does not provide the Clerk’s Office with prepaid postage to the Department of State, certified documents will be mailed back to the student. Send all documents received back from the Erie County Clerk, including the completed Apostille Authentication Request Form, to the New York Department of State.

The mailing address will vary depending on your payment type. Please review the New York Department of State Authentication Request Form for the correct address.

The NY Department of State requires a $10 fee per document. For example, if a transcript and diploma are requested, a $20 fee is payable to the NY Department of State.

Students must include a return mailer to have documents returned to them by mail.

Apostille Process May Take Approximately Five Weeks If Done Exclusively By Mail

Although processing times vary at different offices, it is estimated that this process will take four to five weeks to complete. The apostille process is as follows:

  1. Student sends request for apostille to the Office of the Registrar, transcript request form and copy of diploma to the Office of the Registrar.
  2. UB notarizes document(s) and mails them back to the student; this is the end of processing for the Office of the Registrar.
  3. Student sends documents to the Erie County Clerk’s Office through mail or walk-in service.
  4. Erie County Clerk certifies notarization. If prepaid envelope is provided, documents are sent to the New York Department of State. Otherwise, documents are returned to student.
  5. Documents are sent to New York Department of State either by student or by Clerk’s Office if prepaid envelope is provided.
  6. New York Department of State returns documents with apostille to student.

Request Records of a Deceased Student

In the event of a student’s passing, the University at Buffalo considers the deceased student’s next of kin to be the owner of the record.

All requests for copies of the transcript or diploma of a deceased student must be made in writing, addressed to the Registrar, and include as much of the following student information as possible:

  1. Student’s name while enrolled (please include any known former names, if applicable).
  2. Student ID number (if known), or last four digits of the Social Security Number.
  3. Student’s date of birth.
  4. Student’s dates of enrollment.
  5. Death certificate (photocopy is preferred) or obituary notice; please note that obituary notices can only be used to request a diploma, and we will accept diploma requests from immediate family members; an obituary notice can be used as documentation in lieu of a death certificate if the individual requesting the diploma is listed in the published notice.

The requestor must also provide the following personal information with the written request:

  1. Name of requestor.
  2. Mailing address of the requestor.
  3. E-mail address of requestor.
  4. Phone number of requestor.
  5. Documentation of the relationship of the requestor to the deceased (e.g., birth certificate, marriage certificate, etc.)
  6. Reason for the request.
  7. Signature.
  8. Date of request.
