Forms

The following are various forms students may need throughout their enrollment in the Sustainable Transportation and Logistics program. If a student is in need of ISS-related forms (such as CPT, OPT, or reduced course load requests), plesae send an email to istlgrad@buffalo.edu to discuss further.

Questions and completed forms should be submitted to the graduate coordinator for processing in 118 Bell Hall or via email at istlgrad@buffalo.edu.

Who should file: All STL majors. By the end of the first semester in the program, students should have identified their primary advisor. At this point the student should work with their primary advisor to make out their plan of study for the remainder of their program.  The student should complete an STL Plan of Study Form that is to be signed by their advisor.

When to file: This form should be returned to the Graduate Coordinator in 117 Bell Hall no later than the second week of the student's second semester.

Who should file: STL Majors completing a master's project for their culminating experience requirement. If completing an internship, the offer letter must be attached to the form. The major advisor must sign to acknowledge they approve of the project and will oversee the course requirments/grading. 

When to file: Must submit to the graduate coordinator by the last day of add/drop during your final semester.

Who should file: STL Majors completing a Master's Thesis for their culminating experience requirement. Students must complete with their major advisor and have signed by both their advisor and additional committee members.

When to file: Must submit to the graduate coordinator by the last day of add/drop during your final semester.ester.

Who should file: Master’s students who need to be considered full-time and who are registered for a minimum of one credit hour, but fewer than 12 credit hours (or fewer than nine credit hours with a TA, GA or RA position) and whose department verifies they are working full-time on the master’s capstone requirement, completing final courses, or working full time on an internship related to their program.


When to file
: Prior to the first week of classes for the semester in which the student is to be certified.

Who should file: Students who have completed graduate coursework from another (non-UB) university. 

Transfer Credit Process The director of graduate studies must first formally evaluate any transfer coursework for its applicability toward any particular graduate program of study. Once transfer credits are deemed appropriate and applicable by the department, the student files a Graduate Student Petition for Transfer Credit during the student's first year of matriculation to UB. Upon receipt of the transfer credit petition, the Graduate School will evaluate credit and grade equivalences, and verify the courses were completed at an accredited or recognized institution. UB must receive a final official transcript from the transfer institution before the transfer credits may be formally approved and recorded on the student's UB record.

When to file: During the first semester of the program ideally, but no later than the end of the frist year of matriculation to UB.
