J-1 Scholars are required to confirm their departure date to UBIS at least 30 days prior to the end date of their DS-2019. You need to inform our office by sending an e-mail to immsvcga@buffalo.edu. In the e-mail please provide your full name, SEVIS number, UB department, DS-2019 end date and departure date. If you are departing at least 15 days earlier than your DS-2019 end date you must submit the Early Completion Form. We are required by the U.S. Department of State to update your SEVIS record with your correct end date. The Early Completion Form must be submitted to UBIS at least 30 calendar days before the final date of research activities at UB. If you have not provided to UBIS your most current health insurance information, you will be asked to submit proof that you have maintained health insurance.
