How to Format Your Document

Document Name:

Your document name must include:

  • UB Person Number
  • Last name
  • Type of document

Example: 12345678_Smith_Verification_Form.pdf

If your file is not named in this format, we will not accept the document and it will be discarded. 

Acceptable File Formats:

PDF | JPEG | JPG | PNG | DOCX

We cannot accept the following file formats:

  • HEIC image files—Files taken with an iPhone camera format of High Efficiency and not Most Compatible
  • Zipped files
  • Password protected files
  • Internet files

Form Completion Tips:

  • All Financial Aid form sections must be complete
  • Handwritten signatures are required where indicated (including on tax documents)
  • Submitted documents will not be returned
