We are Interested in Your Thoughts


Breathe Free UB, a committee formed within the UB Office of University Shared Governance and comprised of faculty, staff, students, and community leaders, is interested in the UB community's use of and thoughts about tobacco and other health and wellness behaviors. Below is a link to a brief survey (about 5 minutes) that we hope all students, faculty, staff and alumni will complete. Your answers and comments will help inform UB tobacco policy and other wellness activities. The survey is voluntary and anonymous. Any question may be skipped.

As a thank you for your time, you will have the opportunity to enter a random drawing for one of four (4) $25 Amazon gift cards!

Please note that in order to be eligible for the contest you must:

1. Complete the survey before 12 pm, noon, on Friday, May 12, 2017

2. Provide us with a valid UB email address after you have completed the survey

The email addresses we collect will be only be used for the raffle and will be kept in a file that is completely separate from your survey responses. 

Please help us spread the word and share this announcement widely. Instructors, please encourage students to complete the survey. Employees are invited to share with your colleagues.
