Prehealth Committee Application Checklist

When prepairing your Prehealth Committee Application use this application checklist to ensure you have completed all of the steps for your submission. 

Note: EOPIM applicants should contact a Prehealth Advisor for the EOPIM checklist by October 31st of your sophomore year. This notifies the Prehealth Committee of your intent of application. 

 

Application and Letter Deadlines

  • Spring Application Deadline - March 10
  • Spring Letters of Recommendation (Minimum of 4) Deadline - April 10
  • Fall Application and LOR (Minimum of 4) Deadline - September 1

UB Prehealth Committee Policies

  • Committee letter and individual letters of recomendation will ONLY  be sent to Professional Health Schools
  • Applications submitted with missing information will be considered INCOMPLETE and will NOT be reviewed
  • A nonrefundable application fee will be required after each of the above deadlines - $40.00 for a student applying to any type of Professional Health School

Application Materials Needed By Deadlines

  • Complete all sections
  • Submit the application by the due dates listed above
  • (4-7) Letters
  • (1) Natural  science letter from a PhD level faculty member (Some schools requoire 2) REQUIRED
  • (1-2) Clinically based letter(s) REQUIRED
  • (1) Non-science academic letter (PhD not required)   RECOMMENDED
  • (1) Profesional Reference
  • (1) Research letter recommeded, if participated in research (Required only for those applying to the MD/PhD programs)
  • NOTE: Some osteopathic (DO) schools highly recommend a letter from a DO physician
  • Must be scanned and uploaded to the online application from the University at Buffalo (Found in HUB) - Academic Advising Reports and Transfer Credit Reports are not accepted
  • Include all previously attended colleges
  • Unoficial transcrips must be from the college where credit was earned, including Study Abroad and college courses taken in high school (Not AP credits)

  • Upload a recent headshot of yourself to the application
  • Use a .jpg or .png file no larger than 1 megebyte and must not exceed 200px by 200px
  • File size can be reduced by using a photo resizer app as cropping photos will not reduce the file size
