This form is for State cash receipts only. Cannot be used for Research Foundation (RF), Faculty Student Association (FSA) or University at Buffalo Foundation (UBF) receipts. For UBF cash receipts, use the UB Marketplace Setup for UBF Cash Receipts.
This form is for units that do not have their own UB Marketplace store. If you have an established store in UB Marketplace, new products should be added directly by your store administrato(s).
