UB Marketplace General Store Setup Form for State Funds

Establish a new product in UB Marketplace General Store, a method for collecting payments through state IFR cash receipts. Excludes tuition deposits and graduate application fees.

State Funds Only

This form is for State cash receipts only. Cannot be used for Research Foundation (RF), Faculty Student Association (FSA) or University at Buffalo Foundation (UBF) receipts. For UBF cash receipts, use the UB Marketplace Setup for UBF Cash Receipts.

UB Marketplace General Store Only

This form is for units that do not have their own UB Marketplace store. If you have an established store in UB Marketplace, new products should be added directly by your store administrato(s).


  1. Complete all required fields and click Submit Application. 
  2. A PDF copy of the submission data will be emailed to the contact person's email address provided on the form.
  3. This form must be printed, then signed by your department chair or department head
  4. Obtain the fees group’s signature by emailing the form to ubs-fees@buffalo.edu
  5. Email the signed form to Financial Management, UBS-Marketplace@buffalo.edu

Departments must have an approved fee request form on file prior to establishing an electronic payment. Review Develop and Manage State Fees.
