Submitting Employee Vaccination and Booster Information

As we prepare for the start of the spring 2022 semester, we would like to reiterate that our top priority is the health and safety of our entire university community. The primary way to lower the risk on our campuses is for individuals to be fully vaccinated. As such, UB is updating its guidelines prior to the start of the spring semester, in accordance with new guidance issued by SUNY. This updated guidance requires students to be vaccinated and to have a booster.

Employees are required to provide vaccination information in order to be exempt from weekly surveillance testing. In anticipation of an eventual need to also provide booster information, Human Resources is beginning to collect that information now.

Employees can submit proof of a booster via an improved upload process on the HR website under the COVID-19 Employee Guide. The new upload form, called COVID-19 Proof of Vaccination and Booster for Employees, is linked here and can also be found under the “Quick Links to Forms” section of the guide. Alternatively, individuals can bring their vaccination information card or Excelsior Pass to Townsend Hall on the South Campus, where Human Resources can make a copy. 

Emploees are also advised to continue following the multi-layered protections already in place, which include wearing a high-quality, tight-fitting, high-filtration mask indoors and in public settings; washing hands frequently; staying home when sick; getting tested if you are symptomatic or have been in close contact with someone who is positive; and following recommended isolation and quarantine guidelines.
