Forms for Teaching and Learning Services

Find request forms for faculty and staff engaged in teaching and learning.


Administrative course sites are UBlearns and/or iTunes U sites that are not affiliated with classes offered by registration number. UB faculty or staff may request Administrative sites for research, communication, collaborative or course content development purposes.


Request a UBlearns and/or iTunes U course site combination for courses that have different instructors of record. For example, you and another instructor wish to combine your enrollments to have one course site in UBlearns. Please note that course combination requests can only be accommodated up until the start date of the semester.

Report problems you encounter when you are logged into UB on iTunes U.
Ask a question or report a problem you may be having when using UBclicks.
Request repair or report a problem with classroom technology, the network or telephone service or equipment using this form or contact the CIT Help Desk.

Request the recording of a course or special event in a centrally scheduled or special use classroom.


Request a video or Web conference in one of a few locations on campus.

Report problems you encounter when you are logged into UBlearns.

Request non-standard software installations in technology classrooms by our staff.

UB faculty and staff may request recording capabilities be added or removed for students.  The ability to create a meeting larger than ten attendees may also be added or removed.

Request stand-alone equipment from the list below for use in UB classrooms. You must be a University at Buffalo faculty or staff member to be eligible to order equipment using this form. If you are not affiliated with UB and would like to request this service please call the CIT help desk at 716-645-3542.


Request a combination to unlock a Technology Classroom teaching podium or Video Equipped Classroom video cabinet so you can use the equipment. You must be a University at Buffalo faculty or staff member to receive a combination.


Request space on one of UB's streaming servers to publish streaming media.


You need a username and password to produce live webcasts on one of UB's streaming servers. Request a username if you do not already have one.
