When the event date, time, place and specific details are
finalized, this form will get the process underway to making sure
all the necessary items are available and in place for your
University Facilities charges for the delivery and pick up of the event items listed below. Once the items are delivered, the department or organization are responsible for their safekeeping. The cost for replacement of missing or damaged items will be billed to the department or organization.
