University Facilities employees should contact Customer Service
at 645-2025 to place a work order for safety shoes or
Non-Facilities staff members must send a request to the attention of Customer Service by mail, fax or by using the Customer Service on-line work order request form. The request must include the name of the employee needing shoes/glasses, account number to be charged, authorized signature and a dollar limit. Please indicate on the request if there is no dollar limit. A work order will then be generated.
Work orders can be picked up in the Customer Service office, 120 John Beane Center, North Campus. Once the work order is received, the employee should take it to 143 Beane during the store hours.
