Jefferson Family Medicine Center

A busy, community-based ambulatory clinic in the heart of the city, the new UB practice serves patients in a traditionally underserved, urban neighborhood where disease rates—especially of diabetes and heart disease—are well above the national norms. The clinic is housed in a remodeled, 6,600-square-foot former Rite-Aid building in a small strip mall anchored by a Tops Market.
